Approve model repository code for the cluster

/models/remote-code-approvals/:card_id

Approves one exact signed-registry or content-derived custom-card identity for download and execution across the cluster. The elected master serializes the decision into replicated State; every node then persists its local runner-facing copy. Success is returned only after this API applies the indexed decision. The mutation requires direct loopback access or an authenticated operator-gateway credential.
