Approve or reject one steward action proposal

POST 
/v1/steward/proposals/:proposal_id/decision

Submit one authenticated, single-use decision to the elected master. Approval revalidates expiry, model truth, system-placement protection, and current cluster state before dispatching an existing typed command; a refusal names what changed in the proposal's outcome. Deciding a proposal that already left pending returns 409 with its status and outcome.
