Revoke cluster model repository-code approval

/models/remote-code-approvals/:card_id

Revokes the operator's cluster-wide approval for one immutable model-card identity. Existing processes are not killed, but future downloads and runner starts fail closed. Success is returned only after this API applies the master-indexed decision. The mutation requires direct loopback access or an authenticated operator-gateway credential.
